Which one is correct?
1) you can contact me by email (email address) or by telephone (phone number).
2) you can contact me on email (email address) or on telephone (phone number).
3) you can contact me at email (email address) or at telephone (phone number).

Hello. If you are going to use an email address or a phone number, then at is normal: You can contact me at vitt legal0317 @gmail.com/You can contact me at 1-784-671-2193.

Can I say “you can contact me by telephone at (my phone number) or by email at (my email address)” or saying “by telephone and by email” sounds odd?

Yes, you can say that. Adding by telephone or by email probably isn't necessary, but it isn't weird to do so.

Do I have to use “at” again if I say “you can contact at (my email address) or (my phone number)”? Or I can take it out right before my phone number?

Do you have to use at twice? No, you don't. That said, I would probably use at twice if I needed to give somebody my contact information: You can contact me at 1-715-639-4126 or at owlman5 @gmail.com
